Steps to link your own survey:

  1. Go to https://docs.google.com/forms/d/1QOUHKszSQwH2k3n8ivAmfGeNd1yi4K1eLLwi23wzECA/copy
  2. Click "Make a copy".
  3. Edit the survey to fit your needs.
  4. Click "Publish".
  5. Next to "Responders: Specific people" click "Manage" and adjust the "Responder view" to "Anyone with the link".
  6. Click "Done".
  7. Click "Publish" again.
  8. Click the link icon in the top right corner and select "Copy" to copy the responder link.
  9. Add that link to your "Take the Survey" button URL.
  10. Delete this text box from your email template.
